If you have
been schedule to have a first meeting with a therapist at SunServe’s
facility, you will find it helpful to download these forms, complete
them and
bring them to your first appointment. If you have
difficulty ompleting any section of these forms, you can
discuss that with your therapist at your firstappointment. Complete
what
you can and your therapist will help you with the rest.

with information. It will be helpful for you to
download them and bring them with you as well, but that

describes the way we go about providing care for our clients
and what we expect in return. Bring this to your first
session along with any questions you may have about how we plan
to
work with you.
